Abstract

Cancer constitutes one of the major health challenges of the 21st century, with a sustained increase in incidence associated with population aging and the global epidemiological transition. In cancer treatment, surgery, chemotherapy, and radiotherapy constitute the three fundamental therapeutic pillars of a multidisciplinary approach aimed at disease control and improved survival. However, although radiotherapy is indicated in a substantial proportion of patients with cancer throughout their treatment trajectory, important gaps in access persist, particularly because of delayed referral to radiation oncology. The objective of this brief communication is to synthesize the clinical evidence supporting the main indications for radiotherapy and to provide the medical community at large with a practical framework for identifying patients who may benefit from evaluation by radiation oncology. A narrative review of high-impact clinical studies and international guidelines is presented, demonstrating the benefits of radiotherapy in terms of locoregional control, reduced recurrence, and improved survival in common solid tumors, such as breast, prostate, and rectal cancer. In addition, the main indications for referral are summarized according to tumor location and different therapeutic scenarios. Available evidence shows that omitting radiotherapy when there is an evidence-based indication is associated with worse oncologic outcomes and constitutes a potentially avoidable loss of therapeutic opportunity. In this context, strengthening knowledge of radiotherapy indications among physicians in primary care and other clinical fields, optimizing referral pathways, and expanding the capacity of radiotherapy services are key actions to improve access to effective cancer treatments and reduce avoidable cancer mortality.
